All Y members and Community Participants need to have a completed Participation Agreement and YMCA Association Liability Waiver on file.
Change is inevitable.
Please contact us if you need to update your membership for any reason. For cancellations, we require a completed Cancellation Form to be submitted 14-days prior
to your next draft to discontinue the bank draft and terminate the membership.
The Membership Change form is required for any change to your membership
to take effect. Please complete this form and return to Member Services or email
to firstname.lastname@example.org. Once Member Services receives and processes your Change Form, you will receive confirmation via email or phone. If you do not hear from Member Services within one week of submitting your Change Form, please contact us as we have not received it.
Wisconsin State Law (WI Act 172, Statue 118.293) requires all youth athletic organizations to educate coaches, athletes and parents on the risks of concussions and head injuries. At the beginning of each season, a concussion information agreement must be distributed to parents and coaches.
